Observed employee with left gloved hand grab paper from floor. Employee with contaminated gloved hand took off glove and put on another glove. With same contaminated left gloved hand employee directly proceeded to touch cook food on main cook line. Notified Person In Charge (PIC). Employee reviewed previous steps. Acknowledged touching floor. Employee proceeded to wash hands appropriately at hand sink. ====Employee not washing hands after engaging in other activities that may contaminate the hands.
