To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate anestablishment for which a permit is required without holding the necessary and valid permit to do so.

Please correct violations prior to permit issuance.*** Discussed with PIC cleaning soda nozzles and gaskets shall be with a soap, rinse, submerged in sanitizer for 30 seconds and air dry thoroughly prior to use. Per PIC chlorine sanitizer is used. Verified sanitizer levels of 50ppm to 100ppm shall be used.

Inspector's notes

Observed unit holding several TCS Food without a locking mechanism. Discussed with person in charge (PIC) a functioning locking mechanism is required to protect from offering potentially hazardous food to purchasers. Called and spoke with permit holder and was informed the unit will be swapped out and inspector will be informed of when the new unit is installed. Corrective Action: Correct prior to permit approval.
