Operator could not provide a commissary visit log at time of inspection. Advised operator a commissary visit log shall be recorded, kept on the Mobile Food Trailer, and made available upon request. Corrective Action: Correct prior to next routine inspection. MCEHC Chap. 8, Sec. 3, Reg. 5c - Mobile food establishments shall provide a commissary agreement to the Department upon request, at permit issuance or renewal, and as a part of the documents submitted during plan review for a new permit. Ice cream vendors that only sell or dispense individually packaged, commercially processed items are exempt from providing a commissary agreement. Addl Notes: Mobile food permit was renewed for the month of October. Currently there is no updated commissary agreement on file. Please provide an updated commissary agreement. Corrective Action: Correct prior to next routine inspection.

MCEHC Chap. 8, Sec. 3, Reg. 5e - Mobile food establishments serving time/temperature control for safety foods shall maintain a log of visits to the approved commissary or food service establishment as required by this Department. This log shall be maintained using a Department approved format and maintained with the establishment at all times. The logs shall be maintained for a period of 30 days and should be available for review by the Department.

This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit card can be made in person, via on-line at ESD.MARICOPA.GOV, US Mail, or phone at 602-506-6824 Option 2. Payment can always be made in person at any regional office. We accept cash, check, money order, credit/debit. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. On July 1st, 2023, the Environmental Services Department started emailing permit card(s) and permit invoice(s) to permittees. We need a current and accurate email address on file to ensure you receive a copy of your permit card and invoice(s) upon renewal. Not approved.The following items must be corrected before the issuance of a type 3 permit can occur.1) The commissary agreement submitted is for American Legion Post 35 (permit # FD-02835) with only the food storage box checked. The owner informed me that they planned on disposing the waste water at an RV park. Waste water must be disposed of at a food facility that is your chosen commissary. If your chosen commissary does not have a commissary permit you will need to submit city authorization to dispose waste water from the mobile unit at that food facility. Resubmit commissary agreement with appropriate boxes checked with city authorization. City contact numbers were given to the owner.2) There is a overflow/vent tube going from the top of the waste water tank that feeds downward through the floor of the unit to the outside underneath the unit without a shutoff valve. Vent needs to travel upward so waste water will not overflow through it or it must be capped.No County legal action will result from this inspection.Inspection report printed and handed to the owner.
