Observed tile cove to be missing across near door frame for laundry room ad to the right of the doorway to employee bathrooms. Observed cement floor to be unsmooth and accumulation of broken cement in crack, located in doorway between break room and restrooms. All floors, walls, and ceilings must be maintained and repaired such that it is smooth and easily cleanable. Repair physical facility prior to next routine inspection. Corrective Action: Correct prior to next routine inspection. **Second Consecutive Violation**
