Corrective Action: Corrected at time of inspection. Hose on mop sink faucet extends below the flood rim of the sink below with no back flow prevention device. Manager removed the hose to re-establish the air gap. A back flow prevention device, such as an atmospheric vacuum breaker, is required if a hose is left attached to the faucet.

There was a red cup above the slicer/food prep area. There was no lid or straw. Discussed with person in charge (PIC) that all employee drinks need to be in a designated area, away from food prep areas and that a lid and straw are needed to reduce risk of contamination. Employee moved their drink to designated area and is using a lid and straw. To ensure food safety, prevent physical contamination and the transmission of disease; employee eating, drinking, beverage storage, and using tobacco must be done in an approved area. Corrective Action: Corrected at time of inspection.

Inspector's notes
Observed milk, potato salad, fruit salad, cottage cheese, and cream cheese pastries in a cold-holding unit near the service line with internal temperatures between 43*F and 45*F. Also observed the temperature of the cold-holding unit was 45.1*F using min/max thermometer. The foods that were placed in the unit fewer than 4-hours were place in walk-in cooler. The foods that were placed in the unit more than 4-hours or overnight were voluntarily discard by person in charge (PIC). Please see embargo form. Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. Eggs not treated to destroy Salmonellae can be stored in equipment that maintains an ambient air temperature of 45°F or below. All TCS foods improperly cold held were discarded by PIC at time of inspection. Corrective Action: Corrected at time of inspection.

Discussed with person in charge (PIC) that only a certified pest control specialist shall apply or use pest control. All pest control for the establishment must be performed by a certified pest control specialist and chemicals for pest control must not be stored onsite. PIC removed pest control chemicals from premises at time of inspection. Corrective Action: Corrected at time of inspection.
