The counter tops are in severe disrepair and have a significant break/crack in two separate areas next to the two compartment sink and next to the oven area. The cabinet under the two compartment sink is in disrepair from water damage and has a hole present along the front. Ensure that the cabinets and countertops are replaced and/or repaired properly to a smooth and easily cleanable surface. Physical facility must be maintained in good repair at all times. Observed 0 ppm chlorine sanitizer in dish machine actively being used. Observed empty container of sanitizer. Employee replaced sanitizer and then observed sanitizer not being fed into the machine. Person in charge will put in a work order and an out of order sign on the dish machine. Employee will re-sanitize dishes in the 2-compartment sink and not use the dish machine until chlorine sanitizer is observed at 50-100 ppm. Discussed the importance of proper sanitizer levels and proper monitoring of sanitizer levels. Corrective Action: Corrected at time of inspection.
