Observed pest control snap traps under shelving in dry storage area of office. Pest control methods that are not fully enclosed, concealing deceased pests are not approved. Person-in-charge removed traps from facility at time of inspection. Discussed approved means of pest control with PIC and working with facility's pest control applicator to utilize approved methods. Corrective Action: Corrected at time of inspection.

Observed leaking atmospheric vacuum breaker (AVB) at mop sink in office area. PIC placed work order at time of inspection to have AVB tested and possibly repaired. Discussed importance of backflow prevention devices with PIC. Must repair all plumbing to a state of good repair prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.

Physical facility must be cleaned frequently enough to prevent accumulation of dirt, organic matter, and food debris. Must clean physical facility prior to next routine inspection.
