In the salad bar - Chopped lettuce, chopped cooked egg, sliced cherry tomatoes, diced ham, and other TCS food items (see embargo list) found with internal temperatures of 46-51*F; temperatures verified with PIC's and inspector's calibrated probe thermometers. PIC stated the items were placed in the cooler around 10:00 AM today and hadn't had their temperature checked since that time. PIC discarded the TCS food items in the salad bar. The salad bar showed it was providing an ambient temperature of 47*F. The salad bar will not be used for cold holding TCS food items until it is repaired/replaced and able to hold an ambient temperature of 41*F or below. Please ensure cold held TCS food items are maintained at 41*F or below. Corrective Action: Corrected at time of inspection.

PIC stated an employee had poured oil down the drain which caused it to clog up and have a slow drain. The drain was slow until today when it appears to have been clogged. PIC stated a work order is in place to have the drain cleared. Mop water will be poured down floor sink at 3 compartment sink until the mop sink is repaired. The same type of gap is present on coved base on opposite wall of the office area. FRP wall panel near back door is flexing away from the wall and debris is accumulating behind. Corner panels that have recently been repaired near food prep sink are still missing coved base and the lower internal wall is exposed. Caulking at main hand wash station is beginning to crack away from the wall. Caulking at mop sink has small holes exposing the gap between the basin and wall. PIC stated all the items had been in the cooler since yesterday. PIC was made aware that the cooler was not working properly and ice bathes were being used to cold hold the items in the top portion of the cooler. PIC stated they came into work at 9:00 AM to find all the ice had melted and the internal temperature of the food items were between 44-49*F, and put the items back on ice. Temperatures were verified with both PIC's and inspector's calibrated probe thermometers. Inspector discussed the risk of the possible extended time period over night that the food may have been in the danger-zone and PIC voluntarily discarded the food items (see embargo form).
