Observed T-valve downstream mopsink, attached to chemical dispenser. Advised PIC that this is not approved by the department. PIC disconnected T-valve and chemical dispenser at the time of the inspection. PIC stated a plumber was scheduled to come out. Shut-offs, including chemical dispensers and sprayer nozzles, are not approved for continuous installation downstream from atmospheric vacuum breakers. Observed a chemical dispenser hooked up the mop sink via a T-valve. Informed PIC that the use of T-valves are not permitted and that chemical dispensers must have their own dedicated water supply. The chemical dispenser and T-valve were disconnected at time of inspection. Provided y-valve plumbing handout via email. Observed the establishment sanitizing food contact surfaces with quat sanitizer and no test strips were available onsite at time of inspection. Informed PIC that test strips are needed for any sanitizer used with food contact surfaces. Obtain test strips prior to reinspection. Observed the establishment's only hand sink without adequate hot water supply, measuring at 84*F. Informed PIC that the hot water supply must reach at least 100*F for proper hand washing. PIC fixed the valve and supply adequate hot water at time of inspection. Handwash sinks must be supplied with hot water of at least 100°F, soap and an approved hand drying provision at all times. Facilities must be maintained in a condition that promotes handwashing. Corrective Action: Corrected at time of inspection.

Observed the atmospheric vacuum breaker (AVB) on the mop sink leaking when the water supply is turned on. Informed PIC that a plumber needs to look at the AVB to determine that it is still working.
