Bar dishwasher concentration was still 0 ppm after adjusting cap of sanitizer bottle and priming. Bar manager to take glasses to the back to warewash until repair tech can service machine tomorrow. Maintain 1 inch air gaps above the floor sink. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Equipment inspection.Construction is about 90% MCESD has not authorized the food delivered in the walk-in refrigerator and freezer.Address the following items for final permit approval:(1) Have the Yakatori grills delivered and installed.(2) The air curtains over the bar roll-up must be activated via a micro-switch. They remote control is not acceptable.(3) Provide smooth, vinyl ceiling tile over the bar area.(4) Provide a reduced pressure backflow assembly (ASSE 1013) on the water line isolating the glass rinser at the bar. Route funnel and drain assembly to the floor sink.(5) Provide a minimum 1 inch air gaps for all indirect drains terminating at the floor sinks.(6) Caulk the gaps between the die wall and the counter tops at the bar and kitchen area. Paint the exposed wood edges.(7) Provide a faucet high splash guard on the hand sink as discussed.(8) Touch up kitchen wall paint where needed. Recommend installing FRP for durability.(9) For the small prep sink, provide a hub drain as discussed (diagram provided)(10) Touch up holes/ceiling paint in the employee restroom.(11) Touch up wall paint near the mop sink.(12) If chemical dispensers are installed at the mop sink, provide a separate water line for the connection.(13) Continue all caulking and trim detail work.(14) Provide a final copy of the menu(15) Provide documentation for parasite destruction from food vendor (16) Adjust metered faucets for the restrooms at 95-100 F Please continue construction according to plans, stipulation letter, and inspections.
