Final construction inspection.Construction is complete and approved with conditionsAddress the following items prior to re-inspection:(1) Remove the washer/dryer from the kitchen since the required enclosure has not been provided.(2) Provide dome strainers in all floor sinks.This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. Equipment inspectionConstruction is about 75% completeAddress the following items: (1) Install the sneeze guard system along the front counter. (2) Provide an RPZ backflow assembly isolating each RO water system. No cooper/brass allowed downstream of the RO units.(3) They laundry equipment must be in a sealed enclosure isolated from the rest of the kitchen(4) Have the hood cleaned.(5) Replace yellowed/discolored ceiling tile around the hood.(6) The drop in ice bin is not NSF approved due to internal square corners. Replace with an NSF unit.(7) Additional items may be required once the front counter rinse sink is installed.Please continue construction according to plans, stipulation letter, and inspections. For final approval, have all work completed, have hot water available to all sinks, have all refrigeration maintaining proper temperatures, and have all local municipality approvals.Please call (602) 527-0893 for a final inspection at 100% completion of project or if an additional onsite walk-through is needed.
