Observed the drain at the three-compartment sink overflowing when the compartments drain due to the secondary pipe draining very slowly. Informed PIC that the drainpipe needs to be able to drain without obstruction. Fix prior to next routine inspection. All plumbing must be maintained in good repair at all times. Must repair all plumbing to a state of good repair prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.

To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. If you select payment via US Mail, for further instructions please refer to the printed invoice. No operating permit is valid, until the permit fee is received by the Department. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes.

This inspection was scheduled as a final construction inspection. All items noted in the previous report had been addressed. However, the owner turn-off the breaker for the water heater. Hot water was not provided after 30-40 minutes. Please provide hot water and call to reschedule the final construction inspection.Noted bottle water and bottle drinks are stored inside the restroom. Please do not store foods, drinks, candy, chips, etc. in dirty areas (under the sinks, under drainpipe, inside restrooms, near mop sink, etc.)The report is auto-emailed to the owner. E&D, class 2 0-9Plumbing inspection was conducted with the owners. Construction had been completed. The establishment is open and operating. The owner applied for a retail food permit but they have open food (ice cream). The permit will be changed to E&D, class 2 0-9 seats.Permit is not approved.Please address the following items prior to the final construction inspection:1. Provide a hand washing sink with side splashguards at the front service area. Sink must have hot and cold water.2. Provide hot water for the hand sink inside the restroom.3. Provide a riser or hub drain under the 3-comp sink. Top of the riser or hub drain must be at least 1” below the bottom of the 3-comp sink. Currently the ware washing sink is directly connected to the sewer.4. Provide FRP boards behind mop sink and 3-comp sink. FRP must be from the top of the cove base to 10 inches above the drying rack of the 3-comp sink. 5. Provide drying rack above the 3-comp sink for drying the candy bins. Reviewed how to wash wares with owners.6. There is no certified food protection manager (CFPM) certificate at the site. Per owner, she is a CFPM. The person in charge must be a CFPM and at the site during all operational hours.7. Provide self-closer or self-closing hinges on the restroom’s door. 8. Finish painting the backwall with semi-gloss paint. Use light color only.9. Provide dunnage rack, shelving, or storage cabinets for merchandise, chemicals, tools, and wares.10. The ceiling in the back room where the mop sink, ware washing sink and hand sink are open to structure. Recommend building a soffit over the sink wall to prevent contamination of the sinks from the environment.11. Provide probe thermometer to check food temperature.12. Provide a hot water well for the ice cream scoops. Storing scoops inside the ice cream is not acceptable.No County legal action may result from this inspection.
