Observed low temperature dishwasher in the bar with a chlorine concentration of 0 ppm. Glassware was observed inside the dishwasher at the time of inspection. The dishwasher was ran several times but did not produce the appropriate sanitizer levels. Establishment has a 3-compartment sink and a working dishwasher in the main kitchen. All dishware will be washed, rinsed and sanitized in the main kitchen. Chlorine sanitizer must be between 50-100ppm. Corrective Action: Corrected at time of inspection.

Observed an internal temperature using a probe thermometer of the following foods: Cooked meatballs 45'F, cut leafy greens 48'F - 53'F, pepperoni 45'F, salami 45'F, provolone cheese 50'F, and Italian meat slices 54'F. The left side of the prep cooler was warmer away from the fan and some of the TCS foods were over the fill line. All food above 41'F was moved to the walk-in cooler to cool down. Provolone cheese was voluntarily discarded at the time of inspection. Temperature in the prep table was turned down and discussed not overfilling food on the prep/cook line. Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. Corrective Action: Corrected at time of inspection.

PRIORITY VIOLATION-5-205.15 (A), P: System Maintained in Good Repair; According to Law 1) Provide an approved hand sink connected to hot and cold water in the back of the house. Install hand sink to the left side of the ice machine. Hand sink may drain directly or to the floor sink. Provide splash guards to both sides of the hand sink. 2) Provide an approved hand sink next to the ice cream cooler. Connect hand sink to hot and cold water. Provide a separate rigid drain line for the hand sink. Or all ice cream must be scoped into single service containers in the back of house. 3) Provide an ASSE 1013 RPZ backflow device upstream of the soda tower in the second building. Do not install copper or brass lines/fittings downstream of the RPZ and soda tower. 4) Provide an ASSE 1022 dual check valve backflow preventer upstream of any non-carbonated beverage equipment (i.e. iced tea brewer, ice machine). 5) Reroute the prep sink drain line and water softener drain to the floor sink. Ensure drain line is rigid and has a 1-inch air gap above the floor sink. 6) Reroute the drain line for the dump/rinse sink to the floor sink in the bar. Hand sink drain line can remain direct. 7) Provide internal air gaps (ASSE 1055A) for all chemical dispensers. ASSE 1055B flex gaps are not approved. 8) Provide a 1-inch air gap for all drain lines throughout. Lift all drain lines up off the floor and slope to drain. 9) Repair the hot and cold water handles at the pre rinser. 10) Repair the water leaking from the prep sink faucet. Core-6-501.11, C: Repairing-Premises, Structures, Attachments, and Fixtures-Methods 1) Detail clean throughout the establishment (ceiling tiles, floors, walls, vents, and equipment). 2) Provide new FRP behind the mop sink. Observed FRP to be in disrepair. 3) Provide high temperature silver colored silicone to the gaps of the hood. Remove the old silicone first. 4) Provide a bead of silicone behind all sink fixtures. 5) Provide missing clean out covers and escutcheon plates. 6) Rewrap the pipe behind the condenser fans in the walk-in. 7) Clean and paint the wood behind the prep table with a semi or high gloss paint light in color. 8) Provide a stainless steel cover to the top of the cold plates in the bar. 9) Provide a 3/8” siliconized grout to the wall and floor juncture of the restrooms. 10) Repair the water damaged walls in the second building. 11) Fill in any small holes within the second building. 12) Provide U-shaped toilet seats. 13) Re-attach the splash guard to the hand sink in the expo area. 14) Provide missing/broken floor tiles throughout.
