Observed on site Reduce oxygen packaging (ROP) of halibut fish in the reach in cooler across from the stove. The PIC indicated the fish is vacuumed sealed on site. The one package of fish was discarded at the time of inspection and education was provided regarding ROP process. Discontinue the ROP of fish as it is not allowed. Remove and discontinue the use of the machine and ROP bags. A variance/HACCP must be obtained prior to conducting any specialized processes. Variance/HACCP applications are available through the customer portal "Permit Center" at https://maricopacountyaz-energovweb.tylerhost.net/apps/selfservice#/home. Observed a significant amount of shellstock tags with no final sale date recorded on the tag at time of inspection. Accurate records that are maintained in a manner that allows them to be readily matched to each lot of shellstock provide the principal mechanism for tracing shellstock to its original source if an outbreak occurs. Comment: Shellstock tags must be kept with shellstock until sold, dated with final sale date, and maintained in chronological order for 90 days onsite. Ensure final sold dates are recorded on all shellstock tags. Corrective Action: Corrected at time of inspection.

Final Construction. Walked with owner and contractor. E&D Class 4, 10+ seating.Correct the following items within 30 days:1) Re-locate ice machine in hallway so that it is located inside the kitchen, protected from customers. Ensure drain line from ice machine has a minimum 1” air gap above floor sink.2) Re-locate ice cream freezer so it is against the wall, protected from customers.3) Install an ASSE 1022 backflow preventer on water line to espresso machine.4) Plumb drain line from espresso machine indirectly to the sewer with a minimum 1” air gap. Recommend installing a hub drain at hand sink. Use PVC or other hard pipe for drain line.5) Relocate beer kegerator so that it is near the floor sink. Plumb drain line from kegerator to floor sink in hard pipe with a 1” air gap at floor sink.6) Shorten drain line from 3-compartment sink so that it has a minimum 1” air gap at floor sink.7) Remove old prep sink drain under 3-compartment sink.8) Have RPZ backflow preventer in small bar tested. Maintain certificate on site for inspection. Backflow must be tested and passed yearly.9) Provide a durable non-absorbent surface for all platforms. Recommend using vinyl flooring or similar.10) Fill gaps/rough areas of brick wall in cookline area.11) Seal all small holes in walls with caulk or silicone.This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. If you select payment via US Mail, for further instructions please refer to the printed invoice. No operating permit is valid, until the permit fee is received by the Department.
