Repeat Violation Lettuce and tomato salad, sliced american cheese, and sliced swiss cheese in reach in refrigerator near hand sink with an internal temperature ranging 53-57*F. Person in charge stated food was placed in unit over 12 hours prior to inspection. Foods voluntarily discarded. Observed sliced tomato and cut lettuce in metal tray sitting ontop of a bed ice. Ice was not surrounding tray and internal temperature was 52*F. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services.
