Construction Inspection. Met with Jesus. Follow-up to items from Final. All previously noted items have been addressed.Establishment is not using the former front service area currently, and this was not evaluated during this or the previous inspection. PIC states this area will be remodeled in the future prior to use; ensure the appropriate permits are applied for (either remodel or new plan review - operation permit) prior to inspections and use. Establishment does not have permission to use these areas at this time. Final Construction. Prep kitchen in Sierra Building/Building 3. Walked with Jesus, Marilyn, and William.Correct the following items within 30 days:1) Provide a faucet to 2-compartment prep sink; sprayer alone is insufficient.2) Provide a faucet or sprayer at tilt skillet. If a sprayer is installed, it must either keep an air gap above tilt skillet or approved backflow protection is required.3) Remove unused equipment in ware washing area (old tray line, garbage grinder table, etc.).4) Repair drain in walk-in cooler (from blast chiller) so that it is secured off the floor and sloped to drain.5) Fill gaps in tiles (floor base tiles) inside blast chiller.6) Re-silicone east-side hood throughout, and re-silicone west-side hood where silicone is missing.7) Replace all burnt out bulbs in walk-in units.8) Provide an additional light fixture inside walk-in freezer.This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. If you select payment via US Mail, for further instructions please refer to the printed invoice. No operating permit is valid, until the permit fee is received by the Department.
