Final construction inspection. Construction complete and approved. Before opening please follow up on the following items:1). Install coved base along the floor - wall junctures in the front and at the wall to the side of the mop sink.2). Remove the spray arm assembly on the three compartment sink.This establishment/operation has been evaluated and qualifies for the issuance of the permit. To complete the permitting process, meaning issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Note that fees paid up to this inspection approval did not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Checks and money orders are also accepted via US Mail. If you select payment via US Mail. For further instructions on mail in payments please refer to the printed invoice.Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. First inspection walk trough with the owner. The following items were discussed:1). Ceiling over the work areas will be washable as well as over the mop sink with water heater.2). Coved base will be installed in the back room and front service areas.3). Self closers will be installed for the back door and the restroom door.4). Floor in the back room will be water sealed or painted, please make sure to fill in the control joints.5). Make sure the water heater platform and mop sink cove is finished out. Install FRP in the mop sink cove.6). FRP will be installed on the back wall in the service areas, please provide 8-10 feet in the area with the three compartment and hand wash sink.7). Please make sure to paint any wooden panels on the wall.8). Caulk the control joint in the back wall in the back storage room.9). Make sure the food prep sink is NSF approved.10). Ice machine plumbing will need to be rigid, no flex lines. Install plumbing off the floor and lead to the floor sink following gravity flow and maintain a 1 inch air gap at the sink.Next inspection will be a pre-final when all the finishes are complete.No pitcher rinsers will be installed.Wall damage from the break in will be repaired.No County legal action will result from this inspection.E-mailed inspection report.
