No Certified Food Protection Manager was available onsite at time of inspection. Informed PIC that a CFPM needs to be available onsite during all hours of operation. PIC stated that they are a CFPM but were unable to produce the certificate at time of inspection. A Certified Food Protection Manager (CFPM) is required to be on the premises during all hours of operation. Corrective Action: Correct prior to next routine inspection.

Observed black residue on the inside of the soda nozzles at the self-service soda machine. Informed PIC that the nozzles need to be cleaned and continue to be cleaned at a rate which precludes the growth of organic matter. Discussed that the nozzles are to be treated like food contact surfaces and must be washed, rinsed, and sanitized. All food-contact surfaces must be cleaned to sight and touch. Most items noted in the last report had been addressed.Manager/ the person in charge must be a CFPM and available at work site during operational hours. Maintain all managers’ and food handlers’ certifications on site for inspection purpose.Permit is approved.Please address the following items prior to the next routine inspection:1. Provide drain cup for the vent port of the RPZ backflow preventer of the soda carbonator. Drainpipe of the vent cup must rigid and indirectly discharge to the floor drain with minimum 1” air gap from the top of the hub. 2. Secure the hub drain on top of the floor drain under the soda fountain station.3. Relocate the bag and box location so employee can access the back of the ice machine for cleaning purpose.4. Adjust the drainpipes from the ice machine’s filter and ice bin can be properly discharge the water from the filter and ice bin. Both of the pipes are discharging onto the floor.5. Clean 3-comp and prep sinks and their faucet handles. 6. Clean under the ice machine. There is a layer of black slime on the floor.7. E&D, class 3 10+OC inspection was conducted with the manager in charge, Victoria Romero. The establishment is open and operating.None of the staff is a certified food protection manager (CFPM). Please ensure the person in charge must be a CFPM and available at the site during all operational hours. Food service employees must have food handler certifications. Maintain all managers and food handlers’ certification on site for inspection purpose. Per staff, Jimmy John has their own food handler training program, but no documents can be provided.Permit is approved.Please address the following items prior to the re-inspection on 4/9/24:1. All 3 compartments of the 3-comp sink is leaking water at the drain ports. Please repair/ replace.2. Provide 3/8” radius cove at cabinet and floor junctures of the self-service drink station. Numerous cove tiles are missing or separating from the cabinet.3. Replace the non-testable vented dual check valve (ASSE 1022) for the soda carbonator and the tea brewer is these had not been replaced over a year. This also applies to the water filter for the ice machine. Recommend installing the reduced pressure backflow preventer (ASSE 1013) at the water connection point of the soda carbonator and the tea brewer. This is a testable device, so just test it instead of replacing it.4. Detail clean the facility __floor sinks, ceiling, walls, and floor. There is accumulation of dirt and grime on the floor in kitchen.5. There is a lot of usable and un-usable items stored inside the water heater room. Please go through to organize and store usable properly. Discard un-usable items off the premises to prevent harborage to pests.6. Rearrange the soda bag and box so staff can access the floor sink under the ice machine. There is trash and water pulling under the ice machine due to the floor sink is dirty and blocked by moldy slush. AVB is not design for constant pressure, no shut off valve can be downstream of the AVB. Provide a separate water stub-out with angle stop at the mop sink for chemical dispenser.
