Inspector's notes
At time of inspection, observed the quat test strips and chlorine test strips both soiled and unusable. The establishment uses a quat sanitizer at the 3 compartment sink and a chlorine sanitizer in the dishwashing machine. Working test strips for any sanitizing solution must be kept available at all times. Please obtain test strips prior to re-inspection. Corrective Action: Correct prior to re-inspection.

New owner inspection conducted with owner. Permit approved. Kitchen was not operating at time of inspection. Verified kitchen has one 40-gallon capacity water heater and one 75-gallon capacity water heater. Hot water temperature at handwashing sinks is at least 100 degrees F and at least 110 degrees F at all other sinks.Owner has a temporary handwashing sink for catered events. In addition to temporary handwashing sink, obtain a self-contained portable hand washing station.This establishment is not participating in the award program at this time and had 0 Priority, 0 Priority Foundation violations on this inspection. Emailed inspection report.This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Repair damaged insulation on inside of dock roll-up door. Paint repair with semi-gloss or high gloss paint. Ensure repair and finish is smooth and easily cleanable. 5. Seal, trim, and paint wall and ceiling penetrations in water heater/mop sink area. 6. Reseal metal coved base under hood. 7. Seal gaps on metal coved base outside of wall-in refrigerator. 8. Replace or refinish rusted metal storage racks in walk-in refrigerator. 9. Seal small holes in ceiling of walk-in refrigerator. Ensure repair finish is smooth and easily cleanable. 10. Seal gap between wall and walk-in refrigerator near back exterior door. 11. Refinish rusted coved base in wall-in freezer. 12. Seal around back storage area handwashing sink and install FRP on left wall next to sink. Install FRP on wall around sink. 13. Replace acoustic type ceiling tiles in back storage area with smooth vinyl tiles, or paint tiles with several coats of semi-gloss or high gloss paint. Ensure finish is smooth and easily cleanable. 14. Reseal coved base in male restroom and repair crack floor tile. 15. Provide written operating procedure for cleaning frequency of food transportation containers and catering vehicle. 16. In large catering vehicle, install surfaces that are durable, smooth, and easily cleanable in cargo area where food and utensils will be stored and transported.
