Observed some organic matter on the inside of the soda fountain nozzles. Discussed with person in charge to clean the soda fountain nozzles on a more frequent basis to prevent the accumulation of organic matter inside the soda fountain nozzles. Person in charge will clean the soda fountain nozzles. All food-contact surfaces must be cleaned to sight and touch. Corrective Action: Corrected at time of inspection.

OC inspection conducted with operator Nusrat L.Permit approved.Inspection report emailed to operator.This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. If you select payment via US Mail, for further instructions please refer to the printed invoice. No operating permit is valid, until the permit fee is received by the Department. OC inspection conducted with operator Nusrat Lubna.Permit not approved, operator agrees not to sell TCS food items.Inspection report emailed to operator.Please address the following prior to re-inspection:1. Provide a hand washing sink with metal splahsguards adjacent to the 3-compartment sink.2. Repair the 3-compartment sink drain (was loose).3. Provide an ASSE 1013 RPZ backflow preventer upstream of soda carbonator with relief drain to the floor sink.4. Provide ASSE 1022 upstream of all non-carbonated beverage systems (coffee/tea brew systems).5. Certify all testable backflow preventers and provide copies of the backflow preventer test certifications.6. Repair the leak on the 3-comp sink faucet and mop sink atmospheric vacuum breaker.7. Replace the water heater pan (rusted out).8. Switch the hot and cold-water lines at the front counter hand sink.9. Ensure hot water is provided within 45-seconds to all sinks and service sinks.10. Ensure hot water is provided to all hand sinks measured between 100°F -110°F.11. Ensure the drain lines at the 3-compartment sink are centered and braced.12. Brace all drain lines using uni-strut or equivalent means to ensure drain lines maintain air gap integrity with the floor sink.13. Ensure rigid drain lines are provided for all drip tray systems (soda dispenser), relief drain lines (water filter), and other pieces of equipment used for food items or utensil items (no PEX tubing).14. Remove unused copper drain lines (material may be used for other drains) and cap unused copper line at 3-comp sink.15. Seal all holes, voids, gaps, cracks, and penetrations (electrical and plumbing – soda chase and lines).16. Seal all backsplashes and sides of sink fixtures (3-comp, hand sinks).17. Replace the self-closer for the restroom door.18. Repair the vinyl ACT floor tile around the 3-compartment sink.19. Provide access to soda dispenser floor sink.20. Clean all floor sinks (no longer white).Please be advised that the New Business Permit Application fee includes one initial inspection and one follow-up inspection. Additional fees will be assessed for any additional follow-up plan review inspections.No County legal action will result from this inspection.
