Recommend: replacing internal hanging thermometers inside two door refrigerator unit. Defrost white upright freezer in back room. Of note: Establishment has installed a new Reverse osmosis tank unit. This unit will require a review with Maricopa County Environmental Services Departments. If you keep the tank please contact us at 602.506.6824 email: envplanreview@maricopa.gov within 5 Business days. New plumbing and reverse osmosis unit will require adding an rpz ASSE 1013 backflow preventer on water line.

Of note: at time of inspection the hot water heater was being worked on, by the time inspection was over the hot water was working. Also, establishment took the dish machine off-line and will use the 3-compartment sink moving forward.

Observed a bowl with a ziploc bag of raw chicken, ziploc bag of raw steak, raw ground beef, and a bag of pineapple tid bits in it. Bottom of bowl had liquid from some of the ziploc bags not being closed. Person in charge removed all items and voluntarily discarded the ground beef and pineapple tid bits. Placed the raw chicken and steak in their own container in the refrigerator. Corrective Action: Corrected at time of inspection.

OC inspection for a Class 5 Assisted Living facility was conducted with owner Laksupa C. by appointment. The establishment was closed and not operating at time of inspection. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail.
