Observed multiple floor tiles throughout the kitchen that are cracked and broken. The grout is also damaged in these areas. Discussed with person in charge (PIC) that floors need to be smooth and easily cleanable. PIC stated a work order will be opened. All floors, walls, and ceilings must be maintained and repaired such that it is smooth and easily cleanable. Repair physical facility prior to next routine inspection. Repeat violation: 2nd Consecutive. Corrective Action: Correct prior to next routine inspection.

Discussed with person in charge (PIC) that all cleaners and chemicals need a label to reduce risk of chemical contamination. Observed that the ware wash machine pipes empty below the rim of the floor drains. Person in charge (PIC) stated that the pipes were recently installed.
