Replace table or obtaining corrosion-resistant, nonabsorbent, and smooth material surface for the bottles and container to be stored on prior to the next routine inspection. Observed small countertop fryer in the rear prep area at time of inspection. Establishment does not have a Ventilation Hood System. Fryer needs to be placed under hood, discontinue using the fryer or obtain self contained fryer/ oven. Ventilation hood systems and devices shall be sufficient in number and capacity to prevent grease or condensation from collecting on walls and ceilings. Remove fryer/ obtain different enclosed cooking equipment prior to the next routine inspection. Installing a ventilation hood would require a remodel. Corrective Action: Correct prior to next routine inspection.

According to the person in charge, the Boba was prepared at 12 PM. Person in charge marked the time when the boba should be discarded at time of inspection. Establishment not operating at time of inspection but permit is approved.Correct the noted items prior to re-inspection.This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so.
