Observed food debris on metal tip of can opener and mesh strainers stored away clean. Informed PIC that all food-contact surfaces must be cleaned to sight and touch. All food-contact surfaces that contact TCS foods must be cleaned every 4 hours by washing, rinsing, and sanitizing.PIC relocated all food-contact surfaces to warewashing area to be washed, rinsed, and sanitized at the time of inspection. Observed several snap traps stored underneath the dish washer and inside the dry storage room. Observed rodent droppings inside the dry storage area well. PIC stated that the facility is treated by a pest control company, and that the company checks the traps weekly and only sprays as needed. PIC could not provide any pest control documentation during the time of inspection. Advised PIC of the approved trapping methods and that the rodent droppings must be cleaned up prior to re-inspection on 4/24/26. Also, advised PIC that pest control reports must be provided prior to re-inspection. Corrective Action: Correct prior to re-inspection.

Informed PIC that pest control chemicals cannot be stored onsite and if needed, they must be applied by a certified applicator. PIC removed the chemicals from the premises at time of inspection. All pest control for the establishment must be performed by a certified pest control specialist and chemicals for pest control must not be stored onsite. PIC removed pest control chemicals from premises at time of inspection. Corrective Action: Corrected at time of inspection.

Observed the inside of the ice machine to be coated with a thick white residue and the bottom lip has pinkish organic matter building up. Informed PIC that the ice needs to be burned, and the inside of the ice machine needs to be cleaned and continued to be clean at a rate which precludes the growth of organic matter and residues. All food-contact surfaces must be cleaned to sight and touch. Corrective Action: Correct prior to next routine inspection. PRIORITY VIOLATION-4-501.114 (A-E) - Priority: Manual and Mechanical Warewashing Equipment, Chemical Sanitization- Temperature, pH, Concentration and Hardness. Addl Notes: Observed the mechanical ware washing machine using a 0ppm concentration of chlorine sanitizer to sanitize food contact surfaces at time of inspection. Informed PIC that the machine must be able to pull the correct concentration of sanitizer in order to properly sanitize dishes. PIC agreed to set up the three-compartment sink and wash dishes manually until the machine gets fixed. All food contact surfaces that were washed were moved to the three-compartment sink to be properly washed, rinsed, and sanitized at time of inspection. Discussed the importance of proper sanitizer levels and proper monitoring of sanitizer levels. PIC relocated all food-contact surfaces to ware-washing area to be washed, rinsed, and sanitized at the time of inspection. Corrective Action: Corrected at time of inspection.

Observed the detergent line missing on the dishwasher. Informed PIC that detergent is necessary for proper dish washing and the detergent line needs to be installed prior to reinspection. Observed the machine not dispensing sanitizer properly, pulling a 0ppm concentration. Instructed PIC to use the three-compartment sink for dish washing until the line is installed and both sanitizer and detergent lines are functioning properly. PIC agreed and set up the three-compartment sink at time of inspection. Corrective Action: Correct prior to re-inspection on 03/13/2025.
