Did not observe any live, or deceased, German cockroaches in the previously observed areas of the store. Per store manager, pest control services have performed multiple treatments since the prior inspection and have eliminated the presence of pests. Audited pest control documentation to verify visits. Discussed maintaining the area clean and free of food debris to prevent future harborage conditions with the store manager.

Per staff, this has been an ongoing issue and pest control services visit every 2 weeks to perform treatments. Audited pest control records. Pest control services were at the establishment on 8/8/25 to conduct routine treatments for cockroaches and rodents. Per pest control notes, no cockroaches were observed on 8/8/25. Establishment must be maintained free of pests and all pest control measures must be conducted by a licensed pest control specialist. Corrective Action: Must eliminate current pest control problem and obtain pest control from a certified and licensed pest control specialist by re-inspection. *Reinspection will occur on, or before, Monday August 25th. **3rd consecutive violation

Observed multiple live German cockroaches as well as multiple dead German cockroaches located in the storage compartments underneath the hot roller machines and underneath the storage compartments near the F'Real machine. At the time of inspection, audited the establishments pest control documentation. Pest services were just at the establishment 3 days prior to inspection. Per pest service documentation, no pests were observed and the establishment was treated with traps and insecticides. In addition, observed multiple fruit flies flying in the front corner of the store near the pest service's wall sconce equipment. Establishment must be maintained free of pests and all pest control measures must be conducted by a licensed pest control specialist. Must eliminate current pest control problem and obtain pest control from a certified and licensed pest control specialist by re-inspection. Corrective Action: Correct prior to re-inspection. *Reinspection will occur on, or before, Monday March 24th. *2nd consecutive violation
