Inspector's notes

Observed the display cold holding unit used to store their raw animal proteins having an ambient temperature of 51*F. Establishment had their protocol in place prior to the inspection preventing the sale of all TCS foods in this unit. Technician was able to fix the unit by the end of the inspection and had the unit being able to provide an ambient temperature of 41*F. Reviewed how cold holding equipment must have the capacity to maintain TCS foods at 41°F or below. Corrective Action: Corrected at time of inspection.

Repeat Violation PRIORITY VIOLATION-5-202.11 (A), P: Approved System Observed 4 notch y-valve connected downstream from atmospheric vacuum breaker (AVB) of mop sink in back left corner equipment room without a testable backflow prevention device. Observed two hoses attached to two notches of y-valve with nozzles off as there is a leak from head of faucet that is unable to be stopped with turning off handles of mop faucet. Because of leak and closure of y-valves, constant pressure is being administered to AVB that is not designed to handle the constant pressure. Y-valve was removed at time of inspection. Install testable backflow prevention device such as a reduced pressure zone (RPZ) ASSE 1013 or keep y-valve and other shut off nozzles detached from mop faucet to prevent contamination of water supply. 2nd Consecutive Violation Corrected At Time Of Inspection Core-5-205.15 (B), C: System Maintained in Good Repair; Maintained Observed large leak from head of mop sink faucet when water is turned off in equipment room by restrooms in back left corner of building.
