Third consecutive. Observed orange and tan organic matter buildup on the floor of the walk-in. Also observed a frozen pipe on the cooling unit inside the walk-in with a small leak. A bucket had been placed beneath the leak to collect water as it drips down. Staff stated they have already put in a work order for the leak and frozen pipe. Proper and prompt repair is required. The walk-in is still maintaining food temperatures. Discussed maintaining all surfaces as smooth and easily cleanable. Discussed routine cleaning with staff. Corrective Action: Correct prior to next routine inspection.

Observed an employee with a cut to the center of his palm. The employee had his hand wrapped in gauzes and was unable to wash his wounded hand. Employees with wounds to their hands are restricted from working with food. A second employee arrived during the inspection and will be the only employee working directly with the food. Core-6-501.12, C: Cleaning, Frequency and Restrictions Observed heavy greyish build up underneath the sauce prep table in the main kitchen. Employee stated the prep table is not able to be moved, since the back wheel is broken. Establishment to fix the table so that areas underneath the table are able to be maintained as clean. Observed ceiling grate near the employee handwashing sink covered in heavy amounts of grey dust. All surfaces to be maintained as clean to prevent potential contamination. Also observed heavy matter built up on the floor in the main walk in underneath shelving. Establishment must be cleaned frequently enough to prevent the accumulation of dirt, organic matter, and food debris. Must clean prior to next routine inspection. Core-6-501.11, C: Repairing-Premises, Structures, Attachments, and Fixtures-Methods Observed a rough heavily grooved concrete floor inside the walk in. The flooring was black and grey and was worn down to the aggregate. Proper repair is required. Do not use unapproved methods. Establishment is to create a smooth surface and cover the raw concrete with an approved surface or sealant to make the area non-absorbent, smooth, and easily cleanable. Also observed a door with multiple holes, which had splintering wood fragments extending from the door's surface. Establishment to replace the door with one that is whole, intact, smooth, and easily cleanable. Discussed premises requirements with the manager. Correct By: Correct Prior To Next Routine Inspection
