Observed bag of frozen shrimp, ziplock bag of raw beef and tube of ground beef stored in same container in reach-in cooler thawing. PIC rearranged items at time of inspection into separate containers. All raw animal proteins must be stored below ready-to-eat (RTE) foods and stored according to cooking temperature at all times to prevent cross contamination. PIC properly stored all raw animal proteins according to the final cooking temperature based on the cooking parameters in § 3-401.11 of the Food Code. Observed tofu wrapped in saran wrap cooling in reach-in cooler. PIC uncovered tofu at time of inspection. All TCS foods cooling were uncovered to allow for rapid cooling. TCS foods must be cooled in: uncovered/loosely covered, shallow containers less than 4 inches deep, in equipment designed to rapidly cool, by using an ice bath, by using ice as an ingredient, or another effective means. Predefined Comment: Discussed the importance of proper hand washing and when hand washing is required with the person-in-charge (PIC). Hands must be washed at any time contamination occurs and when changing tasks. Predefined Comment: Discussed the importance of proper hand washing procedures with the person-in-charge (PIC). Proper handwashing procedure is to rinse hands with warm water, scrub with soap for 10-15 seconds, rinse with warm water, dry with approved drying provision, and turn off in a manner to prevent re-contamination of hands. Entire process should take approximately 20 seconds for full and proper handwashing to prevent spread of pathogens and contamination.

Observed an accumulation of dust/debris on walls. Also observed black bacterial growth inside of a reach in refrigerator where a green onion stem was observed to be stuck in the back. All food items inside the unit were packaged. Discussed cleaning inside of the refrigerator routinely, so that food does not get left inside and contaminate other products. Ensure that the shelves get cleaned as well. Corrective Action: Correct prior to next routine inspection.

Priority Foundation-6-501.111 (C), Pf: Controlling Pests; Approved Trapping Methods/Existence Observed rodent droppings in the dry storage room as well as under shelves in other dry storage area. Employees cleaned up most of the droppings. Please clean all areas and sanitize to prevent contamination. Seal all openings in walls/ceilings. Last date of pest control service was for cockroaches in October. Another service is required. Operator is aware of the issue and is scheduling another service prior to the re-inspection date.
