Final Construction inspection conducted with site superintendent. Construction completed and approved. Inspection report was emailed to Mike GueciPlease complete the following items prior to routine inspection:1. Seal tile gaps underneath dining room side countertops2. Seal gaps between stainless steel wall and hoods. 3. Replace ceiling tiles in back kitchen above 3-compartment sink and access door above walk-in refrigerator.4. Finish installing floor tile at base of dining room table booths.5. Provide certificate of occupancy (C of O).6. A certified food manager is required at time of approval. If not already obtained, please obtain your certified food manager certificate from an ANSI approved testing company. All other food handlers must obtain ANSI approved food handler certificates. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Construction inspection conducted with Site Superintendent Mike G. At the time of this inspection, this project was about 75% complete. All major equipment has been installed and finishes have been completed. Establishment is still installing minor equipment, awaiting the carbonated beverage system, some plumbing, and and gas to be installed. Anticipate restaurant opening beginning January 2024.Inspection rights and inspection report was emailed to Mike GueciPlease complete the following items prior to final inspection:1. Provide backflow test report for reduced pressure principle zone assembly (RPZ) backflow preventer, and ensure there are no copper or brass fixtures installed downstream of RPZ.2. Provide ASSE 1022 backflow preventer between water source and any non-carbonated beverage machines such as coffee/tea machines which are directly connected to a water line.3. Provide laminated cleaning procedure from Silikal detailing cleaning regimen for flooring that includes equipment, detergents, sanitizers and emulsifiers.4. Ensure one inch air gaps between equipment drain lines and floor sinks.5. Ensure all floor sinks are 50% exposed for cleaning and routine maintenance.6. Provide adequate weather stripping and door sweeps to all exterior doors. 7. Provide a sneeze guard with side panels for protection of all exposed food along the customer serving counters.8. Conducted onsite visit to asses progress of project with R.S. 1218. Establishment was not open. Looking through windows, equipment is still in packaging and not installed. Called general contractor (GC) for project update. GC stated the project was temporarily suspended due to required electrical system upgrades. GC stated they anticipate resuming work on week of November 13, 2023 and finishing by December 11, 2023. Scheduled equipment inspection with Superintendent on November 28, 2023.Inspection report was emailed to Scott F.
