Construction Re-Inspection. Met with the construction team and golf club management. Correct the remaining items: 1)Provide additional shelving in the beverage/ ice production/ storage rooms. 2)Provide permanent signage for the beverage/ ice production/ storage rooms. 3)Adjust the micro-switch and door so that the micro-switch properly shuts off when not in use. Note: A golf course survey will be conducted in 2024. No County legal action will result from this inspection. Report sent via auto email.:

Final Construction. Met with Jay, Larry, Glenn, and Leo. Correct the following items within 30-days. 1)Provide a lockbox for the air curtain switch. 2)Provide soap and paper towels at hand sinks prior to operation. 3)Clean the ice machines prior to operation. 4)Provide a cleaning policy for the ice machines and handling of ice production/dispensing. 5)Provide hose bib vacuum breakers at hose bibs. 6)Provide a ball valve shut off for the current “dead leg” portion of the water supply line to the water feature fountain. Provide an approved RPZ backflow preventer to isolate the future water feature fountain. 7)Seal all remaining penetrations. 8)Provide a separate dedicated water supply line for the chemical dispenser mounted above the sink in the cart bay. Paint/seal the wood supports. 9)Extend the FRP wall panels to the height of the shelving in the ice machine support room. 10)Provide signage on the room and plumbing.This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. If you select payment via US Mail, for further instructions please refer to the printed invoice. No operating permit is valid, until the permit fee is received by the Department.
