Final construction inspection for a Class 3 10+ E&D permit was conducted with superintendent Rick W. by appointment. The project was 100% complete at time of inspection. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. The inspection was conducted for stock and train approval. The facility is approved to stock and train. Please ensure all items have been corrected from the Equipment inspection conducted on August 8, 2023The final construction inspection is scheduled for Tuesday August 29th, 2023. At the final construction inspection, ensure hot water measuring 120' F minimum is provided to the plumbing system and all refrigeration units are on and measuring at 41' F or below. Equipment inspection for a Class 3 10+ E&D permit was conducted with superintendent Rick W. by appointment. The project is approximately 90% complete. Please ensure the following items are addressed by the completion of the project: 1) Provide a 3/8th inch minimum cove to the floor/wall junctures of the front buffet service areas. Recommend a finger/tool cove with siliconized grout. 2) Provide ASSE 1022 dual checks for non-carbonated beverage equipment. 3) Caulk-in escutcheon plates and plumbing penetrations throughout the facility with silicone. 4) Caulk the sink fixtures and fixed wall shelving units to the walls with silicone. Reviewed room types, hot water system, and laundry facility. The final construction inspection is scheduled for Tuesday August 29th, 2023.
