MOD pizza final inspection. Construction complete and approved. Superintendent will finish up with sealing all base tiles to the floor, front of the house and restrooms. Backflow tested and certified for the soda system.This establishment/operation has been evaluated and qualifies for the issuance of the permit. To complete the permitting process, meaning issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Note that fees paid up to this inspection approval did not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Checks and money orders are also accepted via US Mail. Pre-final walk through with the superintendent. The following items were discussed and will be addressed before the final:1). Floor throughout will be water sealed after cleaning. Especially the new trench back pour.2). There are two areas where the concrete is rough, next to oven and under the butch block bakers table. Please polish out.3). The edge of the power box on the floor under the bakers table will be finished out cleanable.4). If the floor sink next to the table is not used then you will need to install an expandable plug in the drain.5). Sneeze guard is waiting for panels.6). Provide a dual check valve for the tea brewer that meets plumbing standard ASSE1022.7). The glass wall behind the pizza press will be caulked in.8). Here is some paint touch up above the door way to the back room and the soffit above the soda station will need to be painted.9). The bottom rung of the storage racks with food will need to be raised to 6 inches.10). Please seal off all penetration through the walk-in unit, mostly sprinklers.11). One more light will be installed in the back of the walk-in. 12). The base at the front entrance of the walk-in will be finished out where there are gaps.13). A reduced pressure zone backflow valve will be required upstream of the soda system. Install with hub drain and lead drain to the drain in front of the cooler.Do not install copper downstream. Valve will require testing and certification.14). The oven will be flashed to the wall with black metal panels.15). Please install coved base along the soda counter toe kick where the base is not removable.16). The leg collar on the soda station will be mounted.17). The three compartment sink drain is leaking. Please repair.18). Please silicone the inside soda counter cabinets at the shelve and floor junctures to avoid future water damage19). Please touch up the caulk line on the three compartment sink and provide a flush or beveled away from the wall bead of caulk.20). There is one tile missing at the soda counter.21). After cleaning all base tile will be sealed to the floor, throughout.Key is to get City occupancy approval. Once obtained the store will be okay to tock and train next week as long as the occupation does not interfere with the construction. Make sure all soap dispensers have soap in them and you have paper towels at all hand sinks before training.Construction is at 95%. No County legal action will result form this inspection.

First inspection walk through. The following items were discussed:1). Please make sure that the back door door sweep is closed at the threshold and insect tight.2). Restroom doors will receive self closers.3). Base tile where open at the floor juncture will need to be siliconed and sealed to the floor after cleaning, in bathrooms and where occurring. You can also use grout or other sealant. 4). At the soda station in the dining room, provide access to the floor sink and finish out the cut out edges. Make sure the opening is at least the size of the floor sink underneath.5). Install coved base along the toe kick of the soda counter.6). Install a Dual Check valve that meets plumbing standard ASSE 1022 for the Tea Brewer.7). Install a Reduced Pressure Zone backflow valve upstream of the soda system. Install with hub drain and lead drain to a floor sink. Do not install copper downstream and install between 12 inches and 5 feet from the floor.8). The Walk-in will need to be flashed to the walls on the sides.9). Soda counter will be tiled in and counter sealed to the tile.10). Install coved metal base inside throughout the walk-in and along the front on the outside.11). Install an additional light fixture in the back of the cooler.12). Fill in and finish out all penetrations into the walk-in. 13). The prep sink drain comes in dead center, make sure drains line up with drain grate openings so there is no over splash.14). Concrete floor will be patched up where rough and hold in the back room near the mop sink will be backfilled.15). Floors throughout will need to be water sealed.16). Provide split escutcheons at all the drain lines, one noted missing in the restroom.17). Top of the walk-in cooler may stay open if wanted.18). Three compartment sink spray arm assembly will receive a uni-strut.19), Oven will be flashed to the back wall.Construction is at 80%.
