Observed refrigerated unit on line with ambient temperature measured to be 64*F holding Time/temperature Control for Safety (TCS) foods (shrimp 52*F and cooked chicken and cheese rolled tacos 56*F). Per Person in Charge (PIC) items were just stocked in unit at 10:30AM. Items were moved to walk in to facilitate rapid cooling. PIC found that unit was not turned on. Unit was reset and PIC will ensure that it can hold 41*F before TCS foods are allowed to be stored in it. Correct By: Corrected At Time Of Inspection

Final inspection walk-through with superintendent. Construction complete and approved. Superintendent will follow up on:1). Caulk FRP to window frame in waitress station.2). In the men's restroom last stall, seal the schluter to the floor and provide a cleanable surface.3). At the bar, make sure you have at least 1 inch air gaps for all drains, soda gun drain lines need to be properly braced and led to the floor sink aiming down.4). Please caul the ice well in the waitress station just like it was done in the kitchen.5). Uni-struts where loose will be mounted on drain lines.This store will have table service carts. Food will be maintained cold on the cart with hotel pans with lids stored in an ice well.A barrier exemption is not required for this store and bar will be considered an outdoor bar, ice bins have lids and all liquor bottles will have caps.This establishment/operation has been evaluated and qualifies for the issuance of the permit. To complete the permitting process, meaning issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Note that fees paid up to this inspection approval did not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Backflow valve for the glass rinser at the bar will be hooked up.3). Hub drain will be installed for the backflow valve upstream of the soda system.4). Hub drains will be installed for the two backflow valves in the back water heater room.5). Provide an air gap for the condensate line at the mop sink.6). The edges of the ice wells will be siliconed at the counter juncture.7). The window sill in the waitress room will be painted and the edge of the FRP finished out.8). Under the kitchen soda ice well, close hole in the wall.9). All chase lines will be finished out, foam cut back and smoothed out.10). Leaking spray assembly at the mop sink will be repaired.11). Spray assembly at the three compartment sink will be unclogged.12). Door frames at the mop sink will be siliconed to the floor.13). Uni-strut on mop sink faucet will be mounted.14). The chemical dispenser at the food prep sink will require a full air gap in the backflow housing.15). Water line at the hot well, front cooks line will be capped.16). Hand wash sink in staff bathroom will be caulked in, toilet mount screws will be capped.17). Provide brighter lights in the walk-in unit. 18). Top flashing on beer cooler will be finished out.Store has hot water, soap and paper towels have been mounted. All refrigeration is in place and walk-in is on and holding temperature.Stock and train approved after City TCO.No County legal action will result from this inspection.Store will need to apply for outdoor barrier exemption in order to operate the table service carts with open foods.E-mailed report to superintendent.
