Establishment is operating as a Class 4 E&D 0-9 Seating food establishment. Observed multiple foods prepared with advanced cooling processes including various sauces, rice, cooked chicken, as well as various cooked meats from tacos including beef, chicken and beans. Advised PIC a new fee may be assessed on the next billing cycle as well.Observed excessive amount of flour lumps in the chicken batter stored at room temp due to lumps not being sifted out and discarded. PIC stated they wanted to dry out the lumps and blend into a powder again to be re-used. Advised PIC that battering station clumps must be regularly sifted out and discarded. Clumps may not be dehydrated and repurposed.

In the home style fridge labeled "personal food only" observed raw eggs stored above RTE yogurt and raw ginger that was intended for service to customers. All raw animal proteins must be stored below ready-to-eat (RTE) foods and stored according to cooking temperature at all times to prevent cross contamination. Observed an employee wash hands for less than 5 seconds after handling raw chicken. Discussed the importance of proper hand washing procedures with the person-in-charge (PIC). Proper handwashing procedure is to rinse hands with warm water, scrub with soap for 10-15 seconds, rinse with warm water, dry with approved drying provision, and turn off in a manner to prevent re-contamination of hands. Entire process should take approximately 20 seconds for full and proper handwashing to prevent spread of pathogens and contamination. Corrective Action: Corrected at time of inspection. PIC stated the foods were cooked between 9-10am. Except during preparation, cooking, cooling, or when time is used as a control hot held TCS foods are to be maintained at 135*F or above. Since the foods had been out of temperature for less than 2 hours, PIC reheated the chicken in the fryer to 165*F for 15 seconds. Observed the warmer set to setting "8" out of 10. Observed 3 missing heat bulbs on the upper shelf and 2 burned out bulbs above the corndogs on the lower shelf. Advised PIC to turn warmer to highest setting and monitor with a probe thermometer. Discussed storing personal items/foods in areas where they pose no risk of contaminating food or food contact surfaces. Repeat Violation PRIORITY VIOLATION-3-501.16(A)(1), P: Time/Temperature Control for Safety Food, Hot Holding. For a permanent fix to this violation, check out these simple tools in our Active Managerial Control Toolbox, made especially for you. https://www.maricopa.gov/4614/AMC-Toolbox---Hot-Holding. ------ In the open display warmer, observed cooked chicken strips between 116-122*F, a cooked chicken breast at 135*F, and cooked potato wedges between 128-135*F when measured with a probe thermometer. PIC stated the foods were cooked over one hour prior. Except during preparation, cooking, cooling, or when time is used as a control hot held TCS foods are to be maintained at 135*F or above. Since the chicken and potatoes had been out of temperature for less than 2 hours, PIC reheated the chicken in the fryer to 165*F for 15 seconds. Observed the warmer set to setting "6" out of 10 on the upper shelf and between "7" and "8" on the lower shelf. Observed missing heat bulbs on the lower shelf. Advised PIC to turn warmer to highest setting and monitor with a probe thermometer. PIC stated the biscuits burn when on higher settings. Advised PIC to turn off bulbs directly above the biscuits or to store elsewhere to avoid burning. In the steamer, observed the water bath holding at 134*F and TCS foods including potatoes, beans, rice, jambalaya, mac and cheese, and gravy holding between 110-128*F when measured with a probe thermometer. PIC stated the foods had been reheated from frozen in the morning around 8-9am and were voluntarily discarded at the time of inspection (see embargo). Observed the warmer self to "hold." PRIORITY VIOLATION-7-206.11, P: Restricted Use Pesticides, Criteria ------ Observed granular ant killer pesticide applied surrounding the back door of the establishment. Advised PIC all pesticides must be applied by a licensed pest control applicator. Discussed that pesticides may unintentionally contaminate other areas of the establishment including food prep surfaces. PIC removed pesticides at the time of inspection. Priority Foundation-7-202.11, Pf: Restriction-Presence and Use ------ Observed an aerosol can of flying insect pesticide spray. Discussed that only a certified pest control company is authorized to apply pesticides. The PIC removed the pesticides from the building at the time of inspection.
