Final construction inspection for a Class 2 10+ E&D permit was conducted with superintendent John L. by appointment. The establishment was closed and not operating at time of inspection. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Construction inspection for a Class 2 10+ E&D permit was conducted with superintendent John, RS# 1274, and PR Supervisor by appointment. The project is approximately 50% complete at time of inspection. Reviewed sink fixture locations, finishes, and floor sinks. Discussed outer openings. This bar will be permitted as an outdoor bar due to continuous openings. The support kitchen will need to be properly protected with a tight-fitting, self-closing door. Reviewed backflow device install for the rinser as the backflow device will need to be installed horizontally. It will also have a relief port that will need to discharge into a floor sink with an approved air gap. There will need to be accessibility to the backflow device for it to be tested. Please ensure that the panel is easily removable where backflow device will be installed. Staff will also need to easily inspect to ensure that backflow device is not leaking. In addition, please ensure that area where the backflow device is installed is properly finished out. Reviewed boxing out the floor sink. Any curbs will need to be properly coved if not installing coved quarry tile. The curb will need to be sealed prior to any equipment being set on curb.
