To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so.

Observed the main unit holding TCS Food with a malfunctioning locking mechanism and person in charge of permit not being able to show proof the locking mechanism works . Without showing proof of a functioning locking mechanism there is a risk of product becoming out of temperature and failing to automatic shut off via locking mechanism. Please ensure locking mechanism works and being able to prove that in order to get the permit approved. Corrective Action: Correct at once.
