No County legal action will result from this inspection.Of Note: Observed in facility chemical closet a mop sink with a not-in-use chemical dispenser that is directly connected with a hose to a faucet with an AVB. Discussed with Person in Charge (PIC) to have a separate water line installed upstream from the AVB for the chemical dispenser in order to allow the AVB to operate without being under constant pressure. The faucet with the AVB should be able to be used independently of the chemical dispenser in order to prevent chemicals from being syphoned into the clean water supply. Facilities removed the chemical dispenser at the time of inspection. PIC stated a fix was already ordered for maintenance at time of inspection.*******This establishment/operation has demonstrated sufficient compliance to proceed with the permit issuance. To complete thepermitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so.
