Discussed with PIC all food must be made at the commissary or purchased from a permitted establishment. Facilities inspection was conducted at the time of inspection, no food was stored on site. Discussed with PIC: test strips for sanitizer.3 compartment sink is currently being used as hand sink - approved in Nov. 2023. Please ensure only one compartment is used, and other 2 compartments stay covered.

This establishment received a(n) A Grade and had 0 Priority, 0 Priority Foundation on this inspection.Inspection conducted at Mobile Food Office. Inspection report printed and provided at time of inspection. No County legal action will result from this inspection.Okay to permit. Note:Mobile unit will be using one compartment of the 3-compartment sink as handwashing station. Other two compartments will be covered at all times and pluming will have shut off valve. This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit card can be by phone at 602-506-6824 Option 2. Payment can always be made in person at any regional office. We accept check, money order, credit/debit.
