Packaged yogurts, hummus, and milk containers found with internal temperatures of 46-52*F in glass-front cooler in doctor's lounge. The cooler was last known to have a temperature below 41*F at 7:00 AM today. PIC discarded the TCS food items in the cooler (see embargo form). Cut lettuce in ice bath on cafeteria line with internal temperature of 43-52*F. PIC discarded the lettuce. Discussed ice bath construction or possible use of time as a control for cold items near hot well. Please ensure cold held TCS food items are maintained at 41*F or below. Corrective Action: Corrected at time of inspection.

-- Chlorine sanitizer undetectable on dishes ran through low temp sanitizing dishwasher. The dishwasher was primed and reran multiple times and it consistently tested below the 50ppm-100ppm requirement. The PIC stated a work order will be placed to repair dishwasher. PIC relocated all food-contact surfaces to ware washing area to be washed, rinsed, and sanitized at the time of inspection. Of note: A technician arrived onsite during the inspection and repaired the dishwasher. Inspector verified concentration ranged between 50ppm-100ppm at the time of inspection. -- In walk-in freezer, observed several tightly covered hotel pan size containers approximately 6" deep of sliced turkey and buttered corn with internal temperatures ranging from 47°F-53°F using probing thermometer. Per PIC, the items were temporarily removed from the walk-in refrigerator about 1 hour prior to the inspection while employees were putting away delivery. Reviewed approved cooling methods with the PIC: TCS foods must be cooled in: uncovered/loosely covered, shallow containers less than 4 inches deep, in equipment designed to rapidly cool, by using an ice bath, by using ice as an ingredient, or another effective means. All TCS foods cooling were uncovered to allow for rapid cooling. The inspector verified the TCS items were rapidly cooled to 41°F at the time of inspection. Corrective Action: Corrected at time of inspection.

The establishment is using 3-compartment sink with quat sanitizer to wash, rinse, and sanitize until dishwasher is repaired.

-- Observed overfilled containers of diced tomatoes, mozzarella cheese, and sliced deli ham on cafeteria line with internal temperatures ranging from 47°F-56°F. The person in charge (PIC) stated the items were removed from walk-in refrigerator and placed in cold holding unit about 15 minutes prior to the inspection (11:00 AM). At the time of inspection, an employee separated the items in smaller portions and moved them to the walk-in to rapidly cool to 41°F. Discussed ensuring items cold holding with ice baths are completely submerged in the ice. Ensured TCS items are maintained at 41°F or less when cold holding. Corrective Action: Corrected at time of inspection.
