Observed a small puddle of water in front of the three-compartment sink, a blackened floor drain underneath the three-compartment sink, and black and grey organic material on the ceiling in the walk-in. Alerted the manager, who stated they had a work order in for the water, which was coming from a small roof leak. Physical facilities must be maintained in good repair at all times. Repair the physical facility prior to next routine inspection. Physical facility must be cleaned frequently enough to prevent the accumulation of dirt, organic matter, and food debris. Must clean physical facility prior to next routine inspection. Corrective Action: Correct prior to next routine inspection
