Discussed with owner roll up door requirements. ***Establishment must purchase an approved pasteurized mix from a third party provider.Inspection report was emailed to Eric Z.This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select Pay a Fee at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. Equipment inspection conducted with Eric Z. At the time of this inspection, this project was about 90% complete. Inspection report was emailed to Eric Z.***Establishment will be purchasing a pre-made ice cream base from Merit Foods. Correct the following items prior to final inspection:1) Current water heater is not the same water heater on the approved set of plans. Provide the A.O Smith water heater GDHE-50. Provide a recirculation pump on the water heater. All sink fixtures must produce hot water within 45 seconds.2) Provide an air curtain equipped with a micro switch on the roll up door. Light switches and breaker boxes must not be used in place of or circumvent door/window activated micro switches. 3) Provide an ASSE 1015 double check valve upstream of the ice cream maker. Mount the backflow device to the wall. Test and certify the backflow device and provide the test report.4) Provide an ASSE 1022 dual check valve upstream of the ice machine.5) Trim/lift all drain lines so that they have a 1-inch air gap above the floor sink.6) Provide a 1-inch air gap for the dipper well faucet.7) Provide a dedicated water near the mop sink faucet for chemical dispensers.8) Ensure chemical dispensers have an internal air gap ASSE 1055A.9) Provide FRP paneling behind the front bar of the ice cream places. 10) Provide the C of O from the city of Mesa.11) Ensure all cold holding equipment is turned on and holding at an ambient air temperature of 41’F or below. 12) Flash the left side of the mop sink with stainless steel and provide a bead of silicone behind the mop sink.13) Replace the two rows of perforated ceiling tile for washable ceiling tile behind the front ice cream cases.14) Replace the current restroom toilet seats for U-shaped toilet seats.15) Provide a 3/8” cove basin in all the restrooms. 16) Provide light within all the restrooms.17) Ensure all restroom exhaust fans are working properly.18) Provide a standard door for one of the employee restrooms. Ensure doors are self closing.19) Caulk in all gaps and escutcheon plates. Trim the foam and seal over the foam in the walk-in coolers. Fill in any holes with a low temp silicone.Please continue with construction according to plans, stipulation letter, and inspections. Please call (602) 526-0917 for a final inspection at 100% completion of project or if an additional onsite walk-through is needed. Please also ensure you have read your Responsibilities to Qualify for a Permit document and are in compliance with all requirements before final inspection. A certified food manager is required at time of approval. If not already obtained, please obtain your certified food manager certificate from an ANSI approved testing company. All other food handlers must obtain ANSI approved food handler certificates. No county legal action will result from the inspection
