proper cooling for on site prepared food reviewed with manager. noted in the walk-in cooler, pans of onsite cooked refried beans, and pans of on site prepared shredded beef placed under covered multiple stack of plastic food tubs. temperatures for the beans indicating 126 deg f . per manager prepared at noon. also cooked shredded beef deep pan, at 87 deg f . per label made at 1150am. suggested to place cooling food in shallow metal pns. use of ice bath reviewed. once cooling verified, food pans to be covered. manager removed all beans and beef, placed in shallow metal pans. please verify the hot food has reached 41 deg f rapidly. Observed at the cooking area inside the fridge near a handwashing station two metal containers with raw shrimp and bacon. Both containers were stored directly above RTE food such as green salsa and cooked pork. Per PIC, they forgot those two containers were stored there. PIC moved both metal containers with raw protein to the lowest shelf at time of inspection. Corrective Action: Corrected at time of inspection.

Permit is not yet approved.Correct the following items prior to reinspection.1) Replace the hand sink next to the mop sink. Provide a new NSF approved hand sink next to the prep sink. Route the drain line to the nearest floor sink. Do not tie into other drain lines. 2) Repair the walk-in cooler so that it holds an ambient air temperature of 41’F or below. Walk-in unit was not on at the time of inspection.3) Trim the riser for the 3-compartment sink. Riser must be below the bottom basin of the 3-compartment sink. 4) Contact the fire marshal/city of Mesa about the current hood. Ensure hood and ansul systems meet current code. 5) Test and certify both RPZ backflow devices. Ensure drain line for the soda equipment is hard piped to the nearest floor sink. 6) Replace the hand sink faucet for the hand sink at the front of the house. Base of faucet was in disrepair. 7) Ensure all drain lines are hard piped and have a 1-inch air gap above the floor sink. 8) Provide an air curtain to the back delivery door. Air curtain must turn on/off via a micro switch. Repair the air curtain set-up for the drive thru delivery window. Air curtain must not be activated via a light switch. Provide a micro switch. 9) Repair flooring as needed. All flooring must be smooth, easily cleanable and non-absorbent. 10) Switch out the any perforated ceiling tiles in the water heater area and storage room for washable vinyl (light in color). 11) Remove the unused lines near the water heater. Trim and cap off the old chase lines near the water heater. 12) Provide a U-shaped toilet seat for both restrooms.13) Repair or replace the spring for the pre-rinse sprayer for the 3-compartment sink. 14) Trim the foam below the 3-compartment sink and seal over the foam.15) Provide high temperature silver colored silicone to the gaps of the hood.On July 1st, 2023, the Environmental Services Department started emailing permit card(s) and permit invoice(s) to permittees.
