To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit card can be made in person, by US Mail, by or phone at 602-506-6824 Option 2. We accept cash, check, money order, credit/debit. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. The RPZ backflow prevention device for the water supply line to the RO system must be tested and certified and properly mounted on the cabinet wall. Also, provide a funnel drain assembly beneath this device with a hard piped drain line that empties into the floor sink beneath the ice machine with a minimum 1 in. air-gap between the end of the drain pipe and the flood rim of the floor sink.2. There is no hot water at the hand sink in the far west restroom that is also on the back wall of the facility. Provide hot water at the hand sink in the far west restroom.3. Provide minimum 1 in. air-gaps for ends of the three compartment sink and prep sink drain pipes. 4. Caulk and seal the backsplashes of the sinks in the front area to the wall behind them.5. Provide a cove base finish at all floor/wall around the mop sink basin and caulk and seal all spaces between the mop sink basin and the surrounding walls and floor.6. Provide a cove base finish at the floor/wall junctures in the restrooms and provide self-closers for the restroom doors.7. Provide a minimum of a semi-gloss paint finish to any exposed bare wood including the unfinished cabinets around the ice machine and reach in refrigerators in the front area. Also, trim and seal the spaces between the countertops and the cabinets in these same areas.8. Apply more coats of polyurethane to the wood shelves in the front area, including the undersides of these shelves. The wood must be finished to be non-absorbent and easily cleanable.

Provide a certified, drinking water safe, RPZ backflow prevention device for the water supply to the RO system. All piping and fittings on the downstream side of this device must be plastic or stainless steel, no copper or brass.2. Provide an NSF approved food prep/rinse sink with a faucet that has separate hot and cold water valves. This sink must have an indirect drain pipe that empties into a floor sink with a minimum 1 in. air-gap between the end of the drain pipe and the flood rim of the floor sink.3. A 75 gallon capacity water heater is required if a mechanical dishwasher is installed.4. Provide a sneeze guard with side panels that is at least 60 in. high from the floor for all exposed food or drink prep areas along the customer serving counter.5. Provide a cove base finish at all floor/wall and floor cabinet junctures throughout the storage room, kitchen, restroom and mop sink areas.6. Provide light colored, smooth, durable and washable ceiling tiles above the storage room, kitchen restroom and mop sink areas.7. Provide self-closers for the restroom and back doors.8. Provide a minimum of a semi-gloss paint finish to any exposed bare wood including the underside of countertops if necessary.
