Establishment currently has a class 2 retail permit which does not allow any open food handling, including repacking. Spoke with owner and it was decided to switch the permit to a class 3 retail which allows for repacking of items, excluding raw proteins that are not frozen.

Final Construction inspection conducted with site superintendent. Construction completed and approved.Inspection report emailed to Butoyi M.Remaining items will be addressed on RM-10978.This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. If you select payment via US Mail, for further instructions please refer to the printed invoice. No operating permit is valid, until the permit fee is received by the Department. Equipment inspection conducted with Site Superintendent. At the time of this inspection, this project was about 60% complete. Site superintendent acknowledged receiving information on inspection rights.Inspection report emailed toPlease address the following prior to next inspection:1. Provide FRP for the walls inside the warewash room to cover rough walls and CMU blocks.2. Provide smooth easy to clean ceiling in lieu of acoustic tiles, it may be vinyl ACT ceiling tiles or FRP cutout to cover current ceiling tiles.3. Provide an NSF prep sink and ensure it drains into the floor sink. Brace using uni-strut/metal brace to maintain air gap with the floor sink.4. Provide splashguard for the hand sink (left side). 5. Ensure to provide 1-inch air gap for water heater T&P relief drain and overflow pan with mop sink.6. Remove the fan inside the warewash room.7. Seal the juncture between the walls and mop sink.8. Provide self-closer for the restroom door.9. Provide a doorknob for rear exit door.10. Seal all penetrations in restroom and warewash room (electrical and plumbing).11. Provide a cleanout cover adjacent to the mop sink.12. Seal all gaps, cracks, and holes as discussed.13. Provide labels for proportioned items (establishment name, address, and phone number).14. Provide manufacture label for dried fish that come in bulk.Please continue with construction according to plans, stipulation letter, and inspections. Please call (602) 506-6824 for a final inspection at 100% completion of project or if an additional onsite walk-through is needed.
