OC re-inspection for a Class 2 Retail Food Establishment was conducted with owner Sandeep S. by appointment. The establishment was open and operating at time of inspection. Please address the following items prior to the next routine inspection: 1) Continue providing a 3/8th inch minimum cove base to the floor/wall junctures around the three sink area, as needed. 2) Caulk the wall gaps in the restroom, as needed. 3) Caulk-in all escutcheon plates and plumbing penetrations throughout the facility. 4) Provide a hose bib vacuum breaker for the hose bib located next to the mop sink. 5) Provide stoppers for the three compartment sink. Discussed sanitizer test strips and maintaining handwash sinks clear. MCESD inspection report was delivered via email. No County legal action will result from this inspection.

OC re-inspection for a Class 2 Retail Food Establishment was conducted with employee Melissa J. by appointment. The establishment was open and operating at time of inspection. Please address the listed items prior to the next OC re-inspection within 15 days. The establishment has installed a faucet for the three compartment sink that is operational. The facility is approved to sell from the soda fountain. Please be advised that the New Business Permit Application fee includes one initial inspection and one follow-up inspection. Additional fees will be assessed for any additional follow-up plan review inspections.MCESD inspection report was delivered via email. No County legal action will result from this inspection.
1) Provide an additional handwash sink with hot and cold water at the ice machine area or relocate the ice machine to the back area where a handwash sink is installed. If relocated to the back area ensure rigid piping is utilized and indirectly plumbed to the floor sink with a 1" minimum air gap. 2) Provide 1" minimum air gaps above the floor sink flood rim for the three sink drain and the drain lines under the mop sink. 3) Paint the exposed wood substrate and supports throughout the front beverage counter, as needed, with a light colored semi-gloss paint. 4) Provide a 3/8th inch minimum cove base to the floor/wall junctures throughout the restroom and three sink area, as needed. 5) Caulk the wall gaps in the restroom, as needed. 6) Caulk-in all escutcheon plates and plumbing penetrations throughout the facility. 7) Trim the foam sealant around the condenser lines and provide a smooth layer of silicone. 8) Repair the ventilation fan in the restroom. :

OC inspection for a Class 2 Retail Food Establishment was conducted with owner Sandeep S. by appointment. The establishment was open and operating at time of inspection. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. If you select payment via US Mail, for further instructions please refer to the printed invoice. No operating permit is valid, until the permit fee is received by the Department.
