Observed dishes in the mechanical dishwasher in the kitchen with chlorine concentration of 0 ppm. Per PIC, bucket had been switched out this morning. Machine was primed and then after running a dishwasher cycle the final concentration was between 50 and 100 ppm. Discussed testing daily and priming the machine after switching buckets to ensure concentration is between 50 and 100 ppm. Corrective Action: Corrected at time of inspection.

Inspector's notes

Observed cooked garbanzo beans cooling in the walk in refrigerator at 86 degrees Fahrenheit. Beans were finished cooking at 1:00, 3 hours prior to inspection. Per PIC, beans had gone into an ice bath after cooking per procedure but must not have been in there long enough. Establishment did not have a cooling log for verification of times. Beans were discarded at the time of inspection. Cooked foods shall cool from 135 degrees Fahrenheit to 70 degrees F in 2 hours and from 70 to 41 or below in another 4 hours. Corrective Action: Corrected at time of inspection.

Observed dishes in the mechanical dishwasher with chlorine concentration less than 10 ppm. Solution in the bucket was still good, concentration was still less than 10 ppm after priming the machine. Employee who was warewashing filled the 3 compartment sink with quat sanitizer at 300 ppm to sanitize the dishes. Discussed checking the dishwasher daily to ensure the chlorine concentration is between 50 and 100 ppm. Corrective Action: Corrected at time of inspection.

To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. (2) Caulk all holes in the walls, and around all pipe penetrations where needed. (3) Re-caulk the front line hood where it meets the wall. Use metal colored silicone. (4) Adjust all shelving so that the bottom shelf is at least 6 inches above the floor. (5) Repair wall damage/touch up paint in t eh dry storage room and near server area cubbies. (6) Repair wall/base damage at the entrance to the kitchen. (7) Re-caulk the gap between the mop sink basin and the wall. Secure/caulk the loose FRP panels at the back of the mop sink (8) Replace all rusted kitchen shelving (on order) (9) Provide a splash guard on both sides of the back kitchen hand sink. (10) Eliminate rusted surfaces on stainless tables. Ok to use silver Rustoleum paint.
