Inspector's notes
Observed a handwashing sink over by the three-compartment sink without soap available. The manager said they had ordered the soap and were sent the wrong refill for their dispenser. Soap was found and supplied at that sink at time of inspection. Handwash sinks must be supplied with hot water of at least 100°F, soap, and an approved hand drying provision at all times. Facilities must be maintained in a condition that promotes handwashing. Corrective Action: Corrected at time of inspection.

Finish the installation of the second hand sink near the three compartment sink.This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit card can be made in person, by US Mail, by or phone at 602-506-6824 Option 2. We accept cash, check, money order, credit/debit. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. All permit fees are nonrefundable.On July 1st, 2023, the Environmental Services Department started emailing permit card(s) and permit invoice(s) to permittees. We need a current and accurate email address on file to ensure you receive a copy of your permit card and invoice(s) upon renewal. The current chemical dispensers installed in the mop sink room do not have proper air-gaps. All chemical dispensers must have visible air-gaps or a testable, RPZ backflow prevention device must be installed on the water line that feeds them.2. The condensate lines from the outdoor walk-in units must drain to a gravel dry well area. These condensate lines currently drain onto cement slabs behind the units.3. Provide smooth, waterproof ceiling tile throughout the kitchen and dry goods storage room.4. Provide a metal stand for the water heater that allows for cleaning underneath.5. Finish installing the food prep sink, three compartment sink and both hand sinks. All sinks must be secured and sealed to the walls behind each. 6. Ensure that all sinks have hot and cold water available including the mop sink.

Mop sink faucets cannot be used to supply water to chemical dispensers. Provide a dedicated water line for the chemical dispensers in the mop sink area. All chemical dispensers must have visible air-gaps.3. Provide smooth, waterproof ceiling tile throughout the kitchen.4. Provide a cove base finish at all floor/wall junctures throughout the kitchen.5. Ensure that all sinks have hot and cold water available including the mop sink.
