***3rd Consecutive Violation*** Observed employee on main cook line conducting cleaning with rag. Employee then proceeded to directly touch tcs and non-tcs food product with contaminated gloved hands. Same employee touched wrapper from floor with gloved hands. With same gloved hands employee directly proceeded in touching bagels to assemble breakfast sandwich. Notified Person In charge (PIC). PIC directed employee to stop, take off contaminated gloves and wash hands appropriately. Corrective Action: Corrected at time of inspection.

***2nd Consecutive Violation**** Observed cook employee grab contaminated blue glove with clear glove that touched raw beef patties. Cook employee proceeded to directly touch all spatulas and clean hot holding trays with contaminated right gloved hand. Another employee directly touched same contaminated areas on trays. Notified Person In Charge (PIC). PIC directed cook to take off gloves and wash hands. Directed other employee with contaminated clear gloves to take off gloves and wash hands. Observed employee start work from break room touching personal helmet. Employee did not wash hands and started putting on clear gloves. Notified PIC. PIC directed employee to wash hands and put on gloves after. ========Employee not washing hands between working with raw food and RTE food. Employee not washing hands after engaging in other activities that may contaminate the hands. Corrective Action: Corrected at time of inspection.
