Observed cooked beans ready for service. The internal temperature measured 122 degrees F. Instructed PIC to reheat the beans to 165 degrees F and hold at a minimum of 135 degrees F. Observed ground meat at 50*F - 44*F being cooled back to 41*F after grinding in tightly covered plastic containers deeper than 4 inches in depth. As per the owner the meat had been ground up approximately one hour prior to the inspection. Meat was uncovered and placed in shallower containers to cool to 41*F> Corrective Action: Corrected at time of inspection.

Observed both chicken and beef at shawarma equipment at 110*F, with heating elements off. Hot hold of food is to be at 135*F or greater. Equipment must be turned on for rotation and heat. Meat being cooked must be periodically removed and finished cooking at grill and then held warm at 135*F or greater. Operator turned on both the heating element and rotation to operate equipment.
