Plumbing wise, please correct the following items: a). Provide a cleanout cover for the clean out under the three-compartment sink. b). Cap the abandoned air intake flue above the water heater and provide a cleanable surface. c). Remove damaged black caulk and re-caulk the three-compartment sink. d). The water heater condensate is run to the exterior of the structure onto the street. Please see if that can be re-run to the mop sink by means of a sump pump. e). Install a drain line and lead to the floor sink for the counter gutter located next to the ice well in the work counter. Use a frigid drain line where possible and maintain an air gap. Corrective Action: Correct prior to re-inspection.

1). For the physical facility please repair the following items: a). Silicone the splash guard next to the hand wash sink to the counter, both sides. b). Repair the wall under the hand wash / prep sink counter. c). Grout the floor where the grout is recessed. There are few spots. Also grout the floor to the wall at the mop sink before installing coved base. d). Repair the FRP / Wall covering at the corner of the office wall and the wall behind the ice machine. e). Ceiling tile is cut wide at water heater flue, please close the gaps. f). Ceiling is damaged over the ice machine, please replace / repair tile. g). Fill in all the cam holes in the walk-in walls with silicone and finish out smooth. h). Re-mount loose and missing base throughout, kitchen and restroom. Mostly in the dish wash area. i). Also install base along the exterior of the walk-in units. j). Replace open and loose base I the lady’s room. k). The drying rack over the three-compartment sink is coming off the wall, please re-mount. l). Patch and caulk in all the holes in the walls in the north-west corner of the kitchen. Corrective Action: Correct prior to re-inspection. 6-501.12 - Core: Cleaning, Frequency and Restrictions Addl Notes: Clean the following areas: a). The wall and racking over the panini grill and oven. b). Remove dust buildup and clean the fly fan at the back door. Corrective Action: Correct prior to re-inspection. 6-501.114 - Core: Maintaining Premises, Unnecessary Items and Litter Addl Notes: The micro switch on the back door fly fan does not work and the fan is always on. Please repair. Corrective Action: Correct prior to re-inspection.

Provide light shield for the exposed glass light bulbs in the back of the kitchen or install LED light bulbs or replace with LED fixtures. Corrective Action: Correct prior to re-inspection. 6-304.11 - Core: Mechanical-Ventilation Addl Notes: Repair the exhaust fans in the restrooms, both do not work. Corrective Action: Correct prior to re-inspection.
